Responsibilities
- Functional Evaluations: Assess a patient's ability to complete daily tasks like bathing, dressing, grooming and cooking.
- Home Safety Assessments: Inspect the home for fall risk, poor lighting, or tight spaces, and suggest changes like grab bars or ramps.
- Personalized Care Plans: Create clear, measurable therapy goals that fit the patient's home environment and medical needs.
- Caregiver Education: Train family members and hone health aides on safe lifting, transfer techniques, and daily support.
- Adaptive Equipment Training: Teach patients how to use tools like shower chairs, reachers, or custom splints.
- Documentation: Keep accurate medical notes and track patient progress using agency software.
- Master's or Doctoral degree in Occupational Therapy from an accredited program.
- Minimum of 1 to 2 years of clinical experience; home health experience is strongly preferred.
- Active state occupational therapy license and registration with the National Board for Certification in Occupational Therapy (NBCOT).
- Effective written and verbal communication
- Valid driver's license, reliable personal vehicle, and active auto insurance.
- US Citizen or Green Card holder
- Ability to bend, stand, walk and assist with lifting or transferring patients (often up to 50 lbs or more).
